Our staff have knowledge and skills that help us in delivering the necessary care based on assessment of needs. We aim to: Enable service users to remain in their own home at a time of crisis rather than being unnecessarily referred to accident and emergency, residential or nursing home care. Reduce unnecessary or prolonged hospital admission due to delays in access to mainstream domiciliary health and social care services. Promote earlier discharge so that wound care, medications, physical activity and the management of individual nursing needs can continue in the clients own home.
